REQUIRED DOCUMENTS FOR FRESH PASSPORT:
Listed below are the documents required to apply for a Fresh passport.

01] Address Proof: Self –attested copy of any two of the following document(s)

• Aadhaar Card
• Election Identity Card or Valid Driving License
• Electricity Bill (Current Month & Last 1 year back)
• Water Supply Bill (Current & Last 1 year back)
• Bank Passbook or Statement (Last 1 year running)
• Phone Bill
• Gas connection Book
• Rental agreement if staying in a rented house
• Income Tax Assessment order
• Certificate from employer on letterhead with a seal and signature
• Passport of the spouse
• Parents’ passport

02] Age proof- One can submit any of the following documents as age proof for passport:

• School Living Certificate / Secondary school leaving certificate.
• Matriculation or transfer issued by a recognized educational institution/establishment
• Permanent Account Number (PAN) issued by Income Tax Department
• Aadhaar Card/E Aadhar issued by Unique Identification Authority of India
• Driving license issued by the state government's Transport Department
• Voter ID card or Election Photo Identity Card (EPIC) issued by the Election Commission of India
• Service record or Pay Pension Order with DOB (applicable for government employees)
• Policy Bond issued by the Public Life Insurance firms
• A birth certificate issued by designated authority under the Registration of Birth & Deaths Act, 1969

03] 10th and 12th Certificate (for Date of Birth)

04] Degree Certificate or Mark Sheet

05] One Color Photograph

Note: if female Applicant is married then they have to submit Annexure “D” (Affidavit with joint Photograph)

Listed below are the documents required to apply for a re-issue or renewal passport.

Photo- copy of Previous Original Passport (first two pages & last two pages)

• Date of Birth (only for fresh passport)
• Mother/Father Passport or Application receipt
• If Mother or Father have not issued passport, they have to give NOC
• Four Color Photograph (having white background)
• Declaration of Parent/Guardian for Minor Passports (Annexure “H”)

03. Emigration Check Not Required (ECNR) Stamp

• 10th Certificate
• Graduation Degree or Mark-Sheets
• 3 years ITR + PAN Card
• Single Photograph

• Identity Certificate (Annexure "A")
• Verification Certificate as per the specimen (Annexure "B")
• Declaration of Parent/Guardian for Minor Passports (one parent not given consent) (Annexure "C")
• Declaration of Parent/Guardian for Minor Passports (Annexure "D")
• Standard Affidavit (Annexure "E")
• Specimen Affidavit for a passport in lieu of lost/damaged passport (Annexure "F")
• No Objection Certificate (Annexure "G")
• Prior Intimation Letter (Annexure "H")
• A Declaration affirming the particulars furnished in the application about the minor. (Annexure "I")
